The Role
As we expand our High Risk Programs for Seniors, we are seeking passionate and dynamic leaders to oversee the people, processes, and technology that distinguish One Medical.
In the role of Senior Manager for High Risk Care Field Operations, you will report to the Director of High Risk Care Programs and be accountable for fulfilling One Medical's commitment to delivering outstanding quality of care, accessibility, and truly patient-centered experiences at an affordable cost.
Your mission will involve executing high-quality care and service through rigorous performance management and improvement systems.
Specifically, you will oversee the operations of a primary care house calls program and related field services across multiple states, catering to the highest risk patients within our Senior Health population.
Key Responsibilities- Manage daily operations of a program that includes virtual care teams supporting in-home primary care house calls.
- Design and implement standard work processes, collaborating with other leaders to evaluate team results and objectives, while continuously improving operations.
- Oversee risk stratification and patient identification processes to ensure effective enrollment for high-risk patients.
- Ensure patients receive appropriate outreach, Health Coach support, visit scheduling, and care coordination throughout their participation in High Risk Care programs.
- Build an engaged team using effective communication frameworks and Lean principles to achieve patient care and team experience goals.
- Communicate and collaborate with market and in-office care team leaders to ensure clear visibility of performance and effective patient care handoffs.
- Travel to operational locations as needed.
- 3+ years of leadership experience in programs serving high-risk patient populations, particularly in home-based care models.
- 5+ years of experience in operations and process management.
- 3+ years of direct people management experience; virtual team management experience is preferred.
- Deep understanding of value-based care operational contexts and home-based primary care models.
- Proven ability to drive results and support change management and continuous improvement initiatives.
- Strong collaborative skills to foster a supportive and engaged team culture.
- Experience in talent development through mentorship, coaching, and consistent feedback.
- Proficient in data management and analytics tools to maintain program analysis and evaluation.
- Strong written and verbal communication skills.
