Job Summary

We are seeking a skilled and compassionate Registered Nurse to join our team at The Meadows of Leipsic. As a Registered Nurse, you will play a vital role in providing high-quality care to our residents, working collaboratively with our interdisciplinary team to ensure their physical, emotional, and social needs are met.

Key Responsibilities
  • Develop and implement individualized care plans for residents, taking into account their unique needs and preferences.
  • Provide direct care to residents, including administering medications, performing wound care, and assisting with daily living activities.
  • Conduct regular assessments and evaluations to identify changes in resident condition and develop strategies to address any concerns.
  • Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to ensure comprehensive care and support.
  • Communicate effectively with residents, families, and colleagues to ensure seamless care transitions and address any concerns or questions.
Requirements
  • Current RN licensure in the state of Ohio.
  • Minimum of 1 year of experience in long-term care or a related field.
  • Strong communication and interpersonal skills, with the ability to work effectively with diverse populations.
  • Ability to lift, push, and pull up to 25 pounds, with frequent standing, walking, and bending.
  • Flexibility to work a variety of shifts, including evenings, weekends, and holidays.
