Position Overview:
Laurelwood Health Care Center is actively seeking a qualified MDS Coordinator to oversee the Resident Assessment Instrument (RAI) process within our skilled nursing facility. This role is essential in ensuring compliance with state and federal regulations while providing high-quality care to our residents.

Key Responsibilities:
  • Manage the completion of all MDS assessments and related clinical documentation accurately.
  • Develop, implement, and continuously evaluate each resident's comprehensive care plan.
  • Schedule and facilitate interdisciplinary care plan meetings to enhance resident care.
  • Conduct audits of medical records to ensure all items coded on the MDS are supported by appropriate documentation.
  • Provide training and support to department heads, physicians, and staff regarding MDS processes.
  • Collaborate with health plans and the interdisciplinary team to secure insurance approvals and ensure residents meet eligibility criteria.
  • Interpret and apply rules, regulations, and coverage guidelines, serving as a key resource for problem-solving related to the SNF prospective payment system.
  • Utilize the current RAI Manual during the assessment coding process to ensure accuracy.
  • Schedule Assessment Reference Dates (ARDs) for payment assessments, capturing each resident's clinical characteristics.
  • Transmit assessments in accordance with current regulations and confirm the acceptance of MDS submissions.
  • Ensure timely updates of resident care plans in collaboration with the interdisciplinary team.
  • Participate in ongoing education related to the RAI process and complete all required training and competency assessments.
  • Perform administrative duties as assigned, including participation in quality assurance and performance improvement activities.
