POSITION OVERVIEW

Under general supervision, the Physical Therapist is tasked with conducting assessments, diagnosing conditions, and implementing therapeutic interventions in alignment with the organization's policies and the standards set by relevant regulatory bodies. Responsibilities may differ based on specific assignments and program requirements.

KEY RESPONSIBILITIES
The responsibilities of the Physical Therapist at ADAPT MEDICAL PROFESSIONAL SERVICES encompass, but are not limited to the following:
· Conducts comprehensive physical therapy evaluations for individuals on an annual basis, focusing on muscle tone, strength, range of motion, reflexes, gross motor skills, posture, joint alignment, mobility patterns, daily living activities, including transfers and wheelchair usage, and the necessity for adaptive equipment, along with recommendations for modifications to existing tools.
· Formulates a personalized treatment plan and objectives based on the evaluation findings.
· Selects and implements educational, recreational, and social activities to assist clients in adapting to their disabilities and teaches essential adaptive skills for daily living and vocational tasks.
· Collaborates with families and staff to deliver therapeutic services in the home environment of the individuals served.
· Maintains accurate documentation of therapy sessions and outcomes in accordance with organizational policies, contributing to the overall assessment and rehabilitation of clients' medical and psychological conditions.
· Monitors progress on all active therapy cases, updates therapy status, and makes necessary recommendations as conditions evolve.
· Refers clients to additional clinical services when deemed appropriate.
· Adheres to all regulations concerning service delivery and billing practices.
· Performs other related tasks as assigned.

QUALIFICATIONS

· Proficient in reading, writing, speaking, and understanding English.

· Capability to work independently while managing multiple responsibilities.
· Excellent organizational skills.
· Strong oral and written communication abilities.
· Analytical skills necessary for evaluating clients' conditions and progress, and selecting suitable therapy techniques.

EDUCATION AND EXPERIENCE
· Master’s degree in Physical Therapy.
· Minimum of two years of experience working with individuals with intellectual and developmental disabilities.
· IPSIDD approval is preferred.

LICENSE AND CERTIFICATION
Current New York State license in Physical Therapy.
