About the Role

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team in Gallipolis, OH area. As a Homecare LPN, you will provide high-quality patient care to pediatric and adult patients in their homes.

Responsibilities
  • Utilize the nursing process to plan, implement, and evaluate patient care and services provided to clients receiving contracted services.
  • Initiate the plan of care when directed and request revisions as patient status and needs change.
  • Administer medications and treatments as prescribed, and monitor and document effectiveness of interventions.
  • Review plan of care and program protocols to identify areas for healthcare teaching, care coordination, goal setting, and discharge planning needs.
Requirements
  • Current Licensed Practical Nurse (LPN) registration in the state where you practice.
  • Current Health Certificate.
  • Current PPD or Chest X-Ray.
  • Current BLS card.
