Home Health RN Case Manager

Key Responsibilities:
Conduct comprehensive assessments of patients' health status, including physical, emotional, and psychosocial needs.
Develop individualized care plans in collaboration with patients, their families, and the interdisciplinary healthcare team.
Coordinate and facilitate the provision of healthcare services, therapies, and medical equipment as prescribed in the care plan.
Provide patients and their families with education on their medical condition, treatment options, and self-care techniques to enhance their independence and well-being.
Administer medications and monitor medication adherence, side effects, and interactions, ensuring patient safety.
Maintain accurate and up-to-date patient records, documenting all assessments, interventions, and changes in the patient's condition in compliance with regulatory standards.
Regularly evaluate the patient's progress toward their goals and make necessary adjustments to the care plan as needed.
Serve as an advocate for patients and their families, ensuring their needs and preferences are effectively communicated and met within the healthcare system.
Collaborate with other healthcare professionals for the benefit of your patient.
Participate in quality improvement initiatives to enhance the delivery of care and maintain compliance with regulatory standards.
Be prepared to handle medical emergencies, assess the situation, and take appropriate actions to stabilize the patient.
Requirements:
Current RN licensure in state of Indiana
Current CPR certification required
Current Drivers License, vehicle insurance, and access to a dependable vehicle
Strong assessment, care planning, and critical thinking skills
Excellent communication and interpersonal skills
Ability to work independently and as part of a collaborative healthcare team
Strong computer skills for documentation and communication purposes
Ability to lift and move patients and equipment as needed
Comfortable with extended periods of walking, standing, and bending
Adequate vision, hearing, and manual dexterity
