Job DescriptionWe are seeking an experienced Home Health Case Manager to join our team. As a Home Health Case Manager, you will play a critical role in delivering patient-centered care and ensuring the highest level of quality and service to our clients. Your primary responsibility will be to coordinate and provide home health care services to patients in their homes, working closely with other healthcare professionals to develop and implement comprehensive care plans.
Key Responsibilities:- Directly supervise Home Health Aides and LPNs, overseeing their work and providing guidance and support as needed.
- Conduct client assessments, identifying the level of care required and developing personalized care plans with the client and their family.
- Coordinate and deliver home health care services, including medication management, wound care, and rehabilitation therapy.
- Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination.
- Monitor and evaluate patient progress, making adjustments to care plans as needed to ensure optimal outcomes.
- Graduate of an accredited school of nursing with RN licensure in the state where services will be provided.
- One year of home health care experience preferred, with a strong background in case management and care coordination.
- Excellent communication, organizational, and problem-solving skills, with the ability to work independently and as part of a team.
- Competitive salary range: $70,000 - $90,000 per year, based on experience.
