Job Title: Health Coach Specialist

Job Summary:

As a Health Coach Specialist, you will teach, guide, and support individuals with serious mental illness and/or substance use disorders to address barriers in their recovery journey. You will identify and meet clients where they are, coaching them to success and teaching essential skills such as self-advocacy, negotiation, problem-solving, goal-setting, symptom management, and more.

Key Responsibilities:
  • Communicate client status with the interdisciplinary team and referral sources.
  • Participate in team meetings to discuss treatment plans.
  • Complete accurate and timely clinical documentation.
  • Complete authorizations paperwork for clients.
  • Use structured interventions and resources to teach clients.
  • Provide individual and/or group interventions in various settings.
  • Collaborate with community systems to support clients.
  • Assess and update person-centered plans with clients and collaterals.
Requirements:
  • Effectively communicate via written, verbal, in-person, and virtual methods.
  • Customer service skills.
  • Time management skills.
  • Analytical skills.
  • Knowledge of community resources.
