Centerstone is a leading nonprofit behavioral health system seeking a skilled Health Coach Specialist to join our team. As a key member of our interdisciplinary team, you will play a vital role in supporting individuals with serious mental illness and/or substance use disorders to achieve their recovery goals.
Key Responsibilities- Teach and guide clients in developing skills essential for recovery, including self-advocacy, problem-solving, goal setting, and symptom management.
- Collaborate with clients to identify and address barriers to recovery, and develop personalized plans to overcome these challenges.
- Communicate effectively with clients, interdisciplinary teams, and referral sources to ensure seamless care coordination.
- Complete accurate and timely clinical documentation, including ASAMs, Care Plans, and CANS/ANSA assessments.
- Work within various community systems to provide support and resources to clients, including the criminal justice system, child welfare system, and educational institutions.
- Bachelor's degree in a related field or equivalent experience.
- Minimum of two years of experience working directly with adults or youth and families in a behavioral health setting.
- CANS/ANSA Certification required.
- Competitive salary range: $15.75 - $22.00 per hour.
