Job Summary

GuildCare, an innovative community-based Adult Day Health Care Program, seeks a full-time Social Worker to join its multidisciplinary team in its Niagara Falls location. We provide services to adults with chronic medical conditions and vision impairments.

Responsibilities
  • Conduct interviews with prospective registrants and gather necessary data for the admissions process.
  • Develop and implement social work treatment plans for all registrants, re-evaluating psychosocial needs periodically and revising treatment plans as needed.
  • Provide counseling services to registrants and families using individual, family, or group modalities.
  • Advocate for registrants, provide concrete services, and offer information and referral as needed.
  • Track authorizations from managed care organizations and request renewals when due.
  • Lead topical discussion groups and therapeutic activity groups.
  • Develop discharge plans and facilitate referrals and placements.
  • Collaborate with other staff members to address psychosocial and mental health issues.
  • Supervise social work students and volunteers, as needed.
  • Complete all required records and reports in a timely manner.
  • Attend staff meetings and conferences.
  • Work with the team to foster interdisciplinary understanding, cooperation, and a unified treatment approach.
Requirements
  • A Master's degree from an accredited program in Social Work.
  • Prior counseling experience.
  • Proficiency in Microsoft-based programs and strong communication skills.
  • A good working knowledge of local community-based services, the NYS Medicaid System, the DOH Universal Assessment system, and experience in obtaining services through waivered and/or managed Medicaid plans.
  • Experience working with the elderly and/or disabled population, prior experience in Long Term Care, Community Health, or Adult Day Health Care is preferred.
