Job Title: Behavioral Health Care Partners Nurse

We are seeking a skilled and compassionate Behavioral Health Care Partners Nurse to join our team at Bay State Community Services. As a Behavioral Health Care Partners Nurse, you will play a vital role in providing high-quality care to our patients.

About the Role:
  • Collaborate with treatment teams to prepare necessary information for medication reconciliation, including obtaining medication lists, discharge summaries, and other documentations as required in the electronic health record (EHR) system.
  • Communicate directly with patients via telephonic, electronic, or face-to-face interactions, documenting all activities as required in the EHR system.
  • Perform comprehensive assessment reviews, treatment plan/care plan reviews, and sign off on them as part of the multidisciplinary treatment (MDT) team.
  • Request medical records from primary care physicians, higher levels of care, and other collateral sources, and follow up to ensure the MDT receives the records.
  • Adjust medication lists as needed.

Compensation:

The estimated annual salary range for this position is $85,000-$110,000 per year, offering a competitive compensation package.
