Job Description

We are seeking a skilled Behavioral Health Care Partners Program Nurse to join our team at Bay State Community Services. This role requires a unique blend of clinical expertise and administrative acumen.

Responsibilities:
  • Collaborate with the treatment team to prepare necessary information for medication reconciliation, including obtaining medication lists, discharge summaries, and other documentation as required.
  • Communicate directly with enrollees via telephonic, electronic, or face-to-face methods and document all activities in the EHR system.
  • Perform comprehensive assessment reviews and treatment plan/care plan reviews, signing off on them as part of the MDT.
  • Request medical records from PCP, higher-level care, and other collaterals, following up to ensure the MDT receives the records.
  • Adjust medication lists when needed.
Requirements:
  • A valid Massachusetts Registered Nursing License.
  • +2 years of experience working with mental health and substance use disorders.
  • Proficiency in communication technologies, including email, writing skills, cell phone, etc.
  • Knowledge of mental disorders, including SUD, COD, SMI, SED, and physical illnesses, procedures, treatment, related medications, acuity vs. chronicity, and effects on normal growth and development.
  • Demonstrated competency in age-specific behaviors, cultures, effects of illness, and family member's illness on age-related development.
  • Highly organized with the ability to keep accurate notes and records.
  • Experience with EMR systems and working with a multidisciplinary team (MDT).
  • Reliable transportation and a valid MA driving license with an acceptable driving record and current proof of insurance.
  • Experience with transitions of care and familiarity with the system of care.
  • Knowledge of resources for SDOH and the ability to advocate/negotiate systems for and with enrollees.
  • Advanced crisis intervention/treatment/management skills preferred.
  • Strong organizational and communication skills.
  • An acceptable CORI and SORI background record check as required by the program.
