Job Opportunity

We are seeking a highly skilled Nurse Practitioner or Physician Assistant to join our HouseCalls team in this geographic region.

About the Role

This individual will provide annual clinical assessments to patients in the comfort of their own homes as part of the broader Home and Community Care team.

Key Responsibilities:
  • Conduct thorough in-home assessments, including an evaluation of past medical history, review of systems, medication reconciliation, vital signs, and comprehensive physical exams.
  • Perform evidence-based practice screenings, such as point-of-care testing when necessary.
  • IDentify diagnoses to inform care management and active medical management in furtherance of treatment.
  • Formulate a list of current and past medical conditions using clinical knowledge and judgment, along with the findings from your assessment.
  • Communicate assessment findings to inform the member's PCP of potential gaps in care.
  • IDentify urgent and emergent situations and intervene appropriately.
  • Educate members on topics like disease process, medication, and compliance.
Requirements
  • Active NP license or ability to obtain by start date.
  • Active ANCC or AANP national certification in Family, Geriatrics, Adult, Adult-Gerontology Primary Care, or Adult-Gerontology Acute Care certification, or the ability to obtain national certification and/or NP license in state of assignment by start date.
  • Ability to complete physical requirements of the job, including lifting a 30-pound bag in and out of the car, navigating stairs and various dwelling conditions, and sitting, standing, and kneeling to perform physical assessments.
  • Reliable transportation for completing home visit assessments.
Compensation and Benefits

$40,000 Student Loan Repayment or $30,000 Sign-on Bonus for individuals who have not previously participated in this program. Additionally, competitive salary ranging from $125,000 - $175,000 per year, based on experience and qualifications, is offered to successful candidates. Our compensation package also includes access to comprehensive benefits, including medical, dental, and vision insurance, retirement savings plans, and paid time off.
