Overview:

We are creating a new model of care that addresses an individual's physical, mental and social needs.

A competitive compensation package includes $40,000 Student Loan Repayment or $30,000 Sign-on Bonus for Individuals Who Have Not Previously Participated in this Program.

About the Role:

You will provide annual clinical assessments to patients in the comfort of their homes as part of our HouseCalls product.

Key Responsibilities:
  • Conduct comprehensive in-home assessments including medical history evaluation, systems review, medication reconciliation, vital signs, and physical examination.
  • Perform evidence-based practice screenings including point-of-care testing where applicable.
  • Identify diagnoses for informed care management and active medical intervention.
  • Communicate findings with primary care physicians to address potential gaps in care.
  • Respond to urgent and emergent situations appropriately.
  • Educate members on disease processes, medications, and compliance.
Requirements:
  • Holds an active NP license or is able to obtain one by start date. License must be unencumbered, free from disciplinary actions.
  • Has active national NP or PA certification or can obtain it in the state of assignment by start date.
  • Can complete the job's physical demands, including lifting 30-pound bags, navigating stairs, and various dwelling conditions.
