Position Overview

The RN Case Manager is responsible for coordinating and overseeing patient care to ensure optimal quality and cost-effective outcomes. This role involves collaboration with both internal and external interdisciplinary teams. Participation in quality improvement initiatives related to patient care management is essential.

Qualifications

Educational Background:

A Bachelor’s degree in Nursing or a related field is preferred.

Experience Requirements:

Newly graduated nurses, re-entry nurses, and those new to the U.S. healthcare system must successfully complete the CVHP R.N. Residency Program within the first six months of employment. A minimum of two years of recent acute clinical experience is required. Experience in discharge planning, utilization management, and critical pathways is preferred. Strong written, verbal, and time management skills are essential. The ability to work independently while fostering relationships within a team is necessary. Familiarity with medical information systems and excellent customer service skills are mandatory.

Licensure:

A valid California R.N. license is required. BLS certification is mandatory. Annual stroke education is required for those working in specific areas.
