About the Role:

As a Behavioral Health Outpatient Clinician at Andwell Health Partners, you will play a vital role in enhancing the quality of life of our clients by providing innovative and compassionate health care in the comfort of their homes and communities.

Key Responsibilities:

Assess and identify clients' holistic health care needs and concerns as part of a family and community.

Provide psychosocial assessments for clients and develop individualized treatment plans for youth, adults, and families.

Implement patient care plan activities based on treatment goals towards independent living for the patient/family.

Serve as a resource for staff education and training, participate in case conferences as needed.

Requirements:

A Master's degree from a school of social work accredited by the Council on Social Work Education.

Maine Board of Social Work Licensure: LCPC, LCPC-C, LCSW or LMSW-CC.

One or more years of experience as a master's level social worker in a healthcare setting.

A valid driver's license and reliable car with insurance.
