Job Title: Community Outreach Worker

Outreach Development Corp. is seeking a qualified candidate for the position of Community Outreach Worker for our adolescent residential program located in Brentwood, LI.

Job Summary:

The Community Outreach Worker is responsible for coordinating care and participating in discharge planning. Duties include addressing case management needs of adolescent clients and their families, including access to employment, education, entitlements, medical and psychiatric services, housing, socialization, and other supports.

Key Responsibilities:
  • Provide assessment and referral to needed services to support treatment goals and prevent untimely discharge from treatment.
  • Provide clinical intervention, addressing barriers to obtaining needed services.
  • Advocate on behalf of clients to obtain needed services.
  • Coordinate case management of clients with requirements of juvenile justice, foster care, probation, or other mandating agencies.
  • Provide home visits to assist in assessment and engagement in treatment, and to identify barriers to family self-sufficiency.
  • Conduct outreach, develop working linkages, and coordinate with other service providers to assist with client needs.
  • Attend and participate in staff, service provider, and community meetings, where appropriate.
  • Complete all needed documentation of services and contacts in a timely manner.
  • Maintain communication and teamwork with clinical staff to integrate and enhance clinical services.
  • Conduct toxicology tests, as needed.
  • Liaison to district school to provide a seamless transition and support after residential treatment.
  • Exercise due diligence in the delivery of quality care in line with agency Medicaid Compliance Plan and systems of accountability.
  • Ensure communication through the compliance officer and/or the supervisory structure of any violation of non-compliance with the agency's Medicaid compliance plan and personnel policy and procedure.
  • Community management of therapeutic milieu.
  • Group, family, and individual counseling.
  • Discharge planning.
  • Treatment planning, progress notes, probation reports, and other related documentation.
Requirements:
  • CASAC-T, CASAC, LMSW, or LMHC preferred.
  • Experience working with adolescents and their families.
  • Experience working in residential treatment preferred.
  • Experience in marketing a plus.
  • Bilingual Spanish preferred.
  • Commitment to providing quality services and outcome-driven performance measures.
  • Ability to work effectively in collaboration with diverse groups of people.
  • Experience with AVATAR electronic health records a plus.
