We are seeking a skilled Community Habilitation Specialist to provide one-on-one support to individuals with intellectual and developmental disabilities. As a member of our team, you will play a vital role in teaching life skills and promoting community integration.
Key Responsibilities
- Provide individualized support to people with intellectual and developmental disabilities
- Teach life skills and promote community integration
- Collaborate with the team to ensure person-centered plans are implemented
- Document progress and maintain accurate records
Requirements
- High School Diploma or equivalent required; some college preferred
- Experience working with individuals with intellectual and developmental disabilities
- Valid driver's license and reliable transportation
