Position Overview:
The Clinical Care Coordinator is responsible for the execution, management, and oversight of a comprehensive Health, Wellness, and Clinical Program tailored to meet the diverse needs of all Assisted Living Residents, in alignment with Flagship Group's policies and applicable regulations.

Key Responsibilities:
- Conduct thorough nursing assessments, develop care plans, and ensure quality assurance for all residents.
- Maintain accurate and up-to-date medical records, including progress notes and nursing interventions.
- Implement a proactive Wellness Program featuring regular health check-ups for residents.
- Oversee and coordinate with third-party service providers, including but not limited to Podiatrists, Dentists, and Home Health services.
- Serve as the Clinical Leader within the community, advocating for residents and managing care effectively.

Core Duties:
- Uphold Flagship Group's mission in all decision-making processes.
- Assume the role of Case Manager and Delegating Nurse, adhering to regulatory standards.
- Manage all aspects of medication and pharmacy services, ensuring compliance and accuracy.
- Collaborate with the Assistant Director in hiring, training, and evaluating Medication Technicians.
- Ensure adherence to the Nurse Practice Act and relevant regulations in medication administration.
- Review and document physician orders accurately in accordance with company policy.
- Conduct regular checks of medical records to guarantee proper documentation and care quality.
- Engage with residents and their families to discuss health care issues and changes in status.
- Participate actively in care planning meetings and quality assurance initiatives.
- Provide ongoing education and training to staff on clinical practices and nursing standards.

Qualifications:
- Must possess a current nursing license and have graduated from an accredited nursing program.
- A minimum of three years of nursing experience, preferably in geriatric care, is required.
- A Bachelor’s degree in nursing or a related health field with a focus on geriatrics is preferred.
- Completion of the 20-Hour Delegating Nurse Course within the initial employment period is necessary.
