Job Title: Resident Care Coordinator

At Sunrise Senior Living, we are committed to providing high-quality care and services to our residents. As a Resident Care Coordinator, you will play a vital role in ensuring the well-being and satisfaction of our residents.

Responsibilities:
  • Lead the overall operation of the Assisted Living, Terrace Club, and/or Reminiscence Neighborhoods, ensuring compliance with federal, state, and local laws and regulations.
  • Participate in resident evaluations and contribute to the development of resident Service Plans (SP).
  • Maintain appropriate staffing levels and create daily assignments based on resident evaluations and assigned levels of care.
  • Schedule and lead move-in meetings with new residents and families to discuss resident care needs and preferences.
  • Recognize changes in resident condition and take appropriate action, including making notifications as needed.
  • Access and use Sunrise systems, including the electronic health record (Sunrise Care Connect), iCIMS, new resident portal, and OnShift.
  • Participate in the community's weekly Interdisciplinary Team (IDT) meeting to review and discuss selected residents and any changes in care needs or Service Plan interventions.
  • Partner with the Activities & Volunteer Coordinator (AVC) to ensure resident participation in activities.
  • Engage in proactive communication with families on a monthly basis to provide updates on resident condition and life in the community.
  • Communicate with physicians and support services as needed.
  • Develop resident schedules for bathing, laundry, and housekeeping services.
  • Conduct monthly Lead Care Manager Meetings.
Quality Assurance and Regulatory Compliance:
  • Follow federal, state, and local laws and regulations, as well as Sunrise standards, to ensure quality care and service delivery.
  • Participate in QAPI meetings and Performance Improvement Plan committees as required.
  • Maintain a thorough knowledge of current and evolving state laws and regulations, policies, and procedures related to resident care and community operations.
  • Ensure compliance with OSHA requirements.
Financial Management:
  • Explain and discuss the value of Sunrise's products and services for residents, families, team members, and targeted referral sources.
  • Assist the Executive Director in completing the annual community budget.
  • Understand and manage the department budget, including labor and other expenses, and discuss its impact on the overall community budget.
  • Process and submit monthly expenses and budget data timely.
Training, Leadership, and Team Member Development:
  • Perform overall management of the department, including recruiting, hiring, training, coaching, and performance improvement.
  • Provide training to care manager staff on job responsibilities and resident activities of daily living.
  • Partner in the delivery of Sunrise University, onboarding, and annual training.
  • Achieve stated Team Member Engagement goals and lead engagement improvement initiatives as needed.
  • Complete team member staffing and scheduling according to operational and budgetary guidelines.
  • Review timekeeping practices and payroll reports to ensure accurate hours worked and payment.
  • Conduct timely performance appraisals with meaningful conversations.
  • Hold team members accountable and correct actions when necessary, documenting conversations and actions taken.
