About the Role

We are seeking a highly skilled and compassionate Social Work Assistant to join our team at ChristianaCare. As a key member of our care management team, you will play a vital role in supporting patients and their families to navigate the healthcare system and access necessary resources.

Key Responsibilities
  • Provide administrative support to case managers and social workers in discharge planning activities
  • Verify benefits and obtain insurance authorizations for patients
  • Coordinate referrals and orders with vendors and accurately enter data into clinical information systems
  • Collaborate with interdisciplinary care teams to discuss patient discharge plans and needs
  • Communicate with patients, families, and caregivers to ensure smooth transitions of care
  • Perform other related duties as required
Requirements
  • High School diploma or equivalent required
  • Bachelor degree in social work or related field/behavioral sciences preferred
  • Three years of experience in a support role in care management will be considered equivalent to an associate degree in social work or related field/behavioral sciences preferred
  • Home health care and/or managed care experience preferred
  • An equivalent combination of education and experience may be substituted
