About the Role:

This is a critical support role that will assist case managers and social workers in providing exceptional care to patients and their families. The successful candidate will be responsible for verifying benefits, obtaining insurance authorizations, and coordinating post-discharge arrangements.

Key Responsibilities:
  • Provide clerical support to the department, including producing standardized reports and maintaining accurate documentation.
  • Set priorities and demonstrate a strong sense of urgency to meet deadlines.
  • Collaborate with interdisciplinary care teams to discuss patient discharge plans and needs.
  • Ensure authorization is obtained for post-discharge services and follow up with facilities/payers as needed.
  • Perform assigned work safely and adhere to established departmental safety rules and practices.
Requirements:
  • High School diploma or equivalent required.
  • Bachelor degree in social work or related field/behavioral sciences preferred.
  • Three years of experience in a support role in care management will be considered equivalent to an associate degree in social work or related field/behavioral sciences preferred.
  • Home health care and/or managed care experience preferred.
