Key Responsibilities:- Handle initial leads and validate client needs
- Schedule in-person visits to assess client requirements
- Execute service agreements and build rapport with clients and their representatives
- Communicate schedule changes and resolve client issues in real-time
- Collaborate with the Caregiver team to match clients with suitable caregivers
- Participate in weekly client service meetings and contribute to the development of client care plans
- Master's degree in Social Work (MSW) or equivalent
- At least 2 years of clinical experience
- Strong communication and interpersonal skills
- Ability to work in a fast-paced environment and prioritize multiple tasks
- Intermediate computer skills in Microsoft Office products
