Job Title: Registered Nurse - Assertive Community Treatment

Job Summary:

The Registered Nurse (RN) is a vital member of the Assertive Community Treatment (ACT) team at Southeastern Integrated Care LLC. This role requires a skilled and compassionate nurse to provide person-centered services to individuals with serious mental illness, helping them achieve their personal goals.

Key Responsibilities:

  • Manage the medication system in conjunction with the psychiatrist, administer and document medication treatment.
  • Screen and monitor beneficiaries for medical problems and side effects, including nursing assessments and completion of AIMS.
  • Build rapport and engagement with beneficiaries.
  • Engage in health promotion, prevention, and education activities.
  • Develop strategies to maximize medication adherence with beneficiaries.
  • Communicate and coordinate services with other medical providers.
  • Educate the team on monitoring psychiatric symptoms and medication side-effects.
  • Provide crisis response to beneficiaries experiencing a crisis.
  • Attend daily ACT team meetings and actively participate.
  • Prepare and administer oral medications, as well as injectables as prescribed.
  • Supervise LPN staff and accurately document all billable encounters into the EMR system within 24 hours.

Qualifications:

  • Minimum one year of experience working with adults with serious mental illness.
  • Associate's Degree of Nursing for (RN).

Required Skills/Abilities:

  • Working knowledge of psychiatric medications.
  • Ability to empathetically interact with beneficiaries and their families.
  • Strong clinical skills.
  • Operate independently or with minimal supervision.

Certificates, Licenses, Registrations:

  • Valid NC Nursing License or Compact License that allows practice in NC.
  • Copy of a valid, unrestricted NC Driver's license, proof of insurance.

The work is performed primarily in the client's home and in the community, which may involve exposure to cigarette smoke, domestic animals/pets, household pests, uncomfortable heating/cooling, and other issues related to the domestic location in which service is delivered.

The employee must be able to lift and/or move up to 20 pounds and occasionally lift and/or move up to 50 pounds. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus. Must be able to drive and operate a personal vehicle safely and adhere to all applicable state and traffic laws.
