We are seeking a skilled Registered Nurse to join our Assertive Community Treatment (ACT) team at Southeastern Integrated Care LLC. The successful candidate will provide person-centered services to individuals with serious mental illness, helping them achieve their personal goals.
Key Responsibilities- Manage medication systems, administer, and document medication treatment in conjunction with psychiatrists.
- Screen and monitor beneficiaries for medical problems and side effects, including nursing assessments and AIMS completion.
- Build rapport and engagement with beneficiaries, promoting health and well-being.
- Engage in health promotion, prevention, and education activities, developing strategies to maximize medication adherence.
- Communicate and coordinate services with other medical providers, educating the team on monitoring psychiatric symptoms and medication side-effects.
- Provide crisis response and attend daily ACT team meetings, actively participating in team discussions.
- Prepare and administer oral medications, supervise LPN staff, and accurately document billable encounters in our EMR system.
- Minimum one year of experience working with adults with serious mental illness.
- Associate's Degree of Nursing for RNs.
- Valid NC Nursing License or Compact License allowing practice in NC.
- Copy of a valid, unrestricted NC Driver's license and proof of insurance.
The work environment is primarily in clients' homes and communities, with occasional exposure to domestic animals, pests, and other environmental factors.
Physical DemandsThe physical demands of this role include standing, walking, and lifting up to 20 pounds, with occasional lifting of up to 50 pounds.
