Job Title: Registered Nurse

At Southeastern Integrated Care LLC, we are seeking a highly skilled and compassionate Registered Nurse to join our Assertive Community Treatment (ACT) or Family Practice team. As a key member of our team, you will play a vital role in providing person-centered services to our beneficiaries, addressing their diverse needs and helping them achieve their personal goals.

Key Responsibilities:
  • Manage the medication system in conjunction with the psychiatrist, ensuring accurate administration and documentation.
  • Screen and monitor beneficiaries for medical problems and side effects, including nursing assessments and completion of AIMS.
  • Build rapport and engagement with beneficiaries, promoting health and well-being.
  • Engage in health promotion, prevention, and education activities, empowering beneficiaries to take control of their health.
  • Develop strategies to maximize medication adherence, working collaboratively with beneficiaries and psychiatric care providers.
  • Communicate and coordinate services with other medical providers, ensuring seamless care delivery.
  • Educate the team on monitoring psychiatric symptoms and medication side-effects, promoting a culture of excellence.
  • Provide first responder crisis response to beneficiaries in crisis, demonstrating empathy and expertise.
  • Attend daily ACT team meetings, actively participating in decision-making and care planning.
  • Prepare and administer oral medications, as well as injectables as prescribed.
  • Supervise LPN staff, providing guidance and support to ensure high-quality care.
  • Accurately document all billable encounters into our EMR system within 24 hours, ensuring timely and accurate record-keeping.
Qualifications:
  • Minimum one year of experience working with adults with serious mental illness.
  • Associates Degree of Nursing (RN) or equivalent.
  • Valid NC Nursing License or Compact License allowing practice in NC.
  • Copy of a valid, unrestricted NC Driver's license and proof of insurance.
Work Environment:

The work environment is primarily in clients' homes and in the community, with occasional exposure to cigarette smoke, domestic animals/pets, and other environmental factors. The physical demands of the job include standing, walking, and lifting up to 20 pounds, with occasional lifting up to 50 pounds.
