Middlesex Health is seeking a skilled RN Case Manager to join our team. As a RN Case Manager, you will play a critical role in providing case management and education to populations of patients living with chronic disease.
Key Responsibilities- Provide education and resources to patients across the continuum, including inpatient, outpatient, and home settings.
- Act as an adjunct medical and allied health professional in the community, caring for patients with chronic illness.
- Serve as a resource to primary and specialty physicians in identifying and managing high-cost and high-risk patients.
- Bachelor's Degree in Nursing and licensure to practice nursing in the State of Connecticut.
- 7 years of experience in acute care, home care, or long-term care.
- Certification in a nursing specialty.
- Advanced knowledge and understanding of the healthcare delivery system and chronic care populations.
- Strong organizational, communication, problem-solving, and decision-making skills.
- Specialty certification in COPD preferred and/or a minimum of 5 years of experience in managing COPD.
- Masters of Science in Nursing.
- Managed Care experience.
- Case management experience.
