Job Overview

Futures Inc. is seeking a highly skilled and dedicated individual to join our team as a Case Manager. This role is responsible for providing direct support to Consumers in the Middletown area, implementing Behavior Plans, and reporting on progress.

Key Responsibilities
  • Provide direct service to Consumers, including opening and closing buildings, and arranging coverage as needed.
  • Implement Behavior Plans and report on progress weekly.
  • Report to Supervisor on Consumer needs and organize household materials, including lifting of household items.
  • Participate in individual team meetings as needed.
  • Develop communication strategies and answer questions from Consumers.
  • Comply with Federal, State, and Local legal requirements by enforcing adherence to requirements.
  • Represent the organization by attending L10 meetings and participating in individual team meetings as needed.
  • Achieve organization's financial objectives by confirming staff itineraries and verifying completion of billable hours upon absence of supervisor.
  • Contribute to the organization's effectiveness by collecting, updating, interpreting, reporting, and distributing data related to all participants and their services.
  • Ensure that issues of performance accountability are routed to the appropriate supervisor on a timely basis.
  • Supervise staff by overseeing and conducting training, including orientation for new and existing staff.
  • Maintain professional and technical knowledge by attending educational workshops, reviewing professional publications, establishing personal networks, and participating in professional societies.
  • Identify issues and trends that require systems improvement.
  • Perform office-related tasks to ensure operational goals are met.

Requirements

  • Bachelor's Degree preferred, or equivalent experience required with specialization in psychology, special education, counseling, youth work, or equivalent life experience.
  • Two to five years' experience working with Consumers with developmental disabilities, including experience with behaviorally challenged Consumers.
  • Good independent assessment, observation, and organizational skills.
  • Excellent computer skills and the ability to operate computer programs (e.g., Word, Excel, Medisked) and assistive technology devices (e.g., iPad).
  • Good interpersonal skills in a variety of settings and situations.
  • Ability to effectively present oneself, orally and in writing.
  • Ability to exercise discretion and work independently with general supervision.
  • Reliable transportation; required to travel between multiple offices.

Salary and Benefits

Salary: $23.00 hourly. Period of employment: Full-time Hourly (35 hours), non-exempt position.
