About the Role:

The Supported Housing Case Manager will provide housing and support services to chronically homeless individuals and families. This position requires a strong understanding of case management principles and the ability to work with individuals with complex needs.

Key Responsibilities:

  • Engage with recipients to create a shared Housing Health and Recovery Plan
  • Provide outreach and assistance securing apartments in affordable housing complexes
  • Offer services that touch all life domain areas, including employment, finance, and medical needs
  • Partner with recipients to integrate into their community of choice
  • Assist with discharge planning and ensure a smooth transition to independence

Requirements:

  • 2 years of experience in case management or mental health services
  • Excellent interpersonal and engagement skills
  • Very good organizational skills
  • BA required, minimum 1 year experience
