Job Title: Pediatric Home Health Nurse

Job Summary:

Thrive Skilled Pediatric Care is seeking a skilled and compassionate Registered Nurse to join our pediatric home health team. As a Pediatric Home Health Nurse, you will provide high-quality clinical care to medically fragile children in the comfort of their own homes.

Responsibilities:

  • Conduct thorough home health assessments on newborns to teenagers
  • Develop and implement individualized care plans to meet the unique needs of each patient
  • Provide education and training to patients and their families on disease management and self-care techniques
  • Monitor patient progress and adjust care plans as needed
  • Collaborate with other healthcare professionals to ensure comprehensive care

Requirements:

  • Current and unencumbered state licensure as a Registered Nurse
  • Minimum 1 year home health experience
  • Minimum 1 year recent clinical nursing practice, preferably in ICU or pediatrics
  • Experience with NG or GTube feedings and ostomy care (preferred)
