Medical Care Coordinator

FRIENDS of Broomfield is seeking a dedicated and passionate RN professional to oversee the medical care of adults with intellectual and developmental disabilities in our Residential Program.
The RN will provide person-centered health care management services by coordinating and overseeing individualized health care plans that include all essential services to meet the specific medical needs of the individual.
Key Responsibilities:
- Coordinate care with the person's team
- Monitor medication and health issues of individuals served
- Consult with directors, program managers, staff/DSP's regarding specific needs of the adults as well as general medical issues
- Oversee the administration of medications, as needed
- Provide assessments for individuals to rate their knowledge and ability to self-medicate
- Assist in accessing services and coordinating care
- Routinely review Medication Administration Records (MARs) for adults served and physician orders and update as needed
- Develop, implement, and train staff on medical protocols, health care plans, and review and revise as needed
- Advocate for the best interest of the person by working with providers as it relates to the individual's care
- Ensure effective communication between each residential setting, family/guardians, individuals served, and medical professionals where required
- Provide training for providers and employees on health and medical related issues
- Ensure compliance with regulatory agencies
- Document timely utilizing standards of practice for the profession
Requirements:
- Colorado Nursing License is current and in good/active status
- Three years' experience working with adults with intellectual and developmental disabilities
- Good written and spoken language skills
- Able to maintain detailed documentation, records and reports
- Valid Colorado driving license, good driving record and reliable transportation
