We are seeking a highly skilled and experienced Care Management Director to lead our Inpatient Care Management team. The successful candidate will be responsible for providing strategic leadership and oversight of the care management program, ensuring high-quality patient care and efficient operations.
Key Responsibilities- Develop and implement care management strategies to improve patient outcomes and reduce readmissions.
- Lead a team of care management professionals to ensure timely and effective discharge planning and coordination.
- Collaborate with hospital leaders, physicians, and other stakeholders to ensure seamless care transitions and optimal patient outcomes.
- Monitor and analyze data to identify trends and areas for improvement, and develop strategies to address these issues.
- Ensure compliance with regulatory requirements and accreditation standards.
- Master's degree in Nursing, Social Work, or related field.
- 5+ years of experience in care management or a related field, with at least 1 year of leadership experience.
- Strong leadership and communication skills, with the ability to motivate and inspire a team.
- Ability to analyze data and develop strategies to improve patient outcomes.
- Knowledge of regulatory requirements and accreditation standards.
- Experience with electronic health records and care management software.
- Knowledge of population health management and value-based care.
- Certification in case management or a related field.
