Job Title: Refugee Group and Education Coordinator

Community of Hope is seeking a highly motivated and experienced Refugee Group and Education Coordinator to join our team. As a key member of our healthcare team, you will be responsible for promoting, facilitating, and leading health education workshops, socialization/wellness groups, and other group education offerings for refugees and underserved populations.

Key Responsibilities:
  • Implement health education workshops using health literacy best practices and cultural relevance.
  • Design and facilitate specialized ad hoc groups and/or individualized education interventions to reduce gaps in education for high-risk or priority patient populations.
  • Revises existing and develops new education materials in collaboration with Refugee Health Care Coordinators, Population Health leadership, and clinical leadership to support educational interventions.
  • Collaborate with clinical, Quality Improvement (QI), and Population Health leadership to plan, implement, and evaluate innovative approaches to group educational support for patients.
  • Ensure completion of pre and post evaluations by workshop participants to evaluate participant knowledge retention and satisfaction.
  • Develop quarterly group offering calendars in collaboration with supervisor, facilitators, and Refugee Health Care Coordinators.
  • Ensure proactive communication of calendar changes to supervisor, COH staff, and group participants.
  • Develop and distribute promotional materials for all group offerings, for the purpose of recruitment.
  • Collaborate with the QI team to develop Relevant reports that support evaluation of group reach and impact.
  • Facilitate and/or participate in case conferencing and warm handoffs with the care and support teams of program participants and patients to ensure adequate and meaningful connection to educational intervention and care.
  • Submit order requests for group supply inventory (to include incentives, activity materials, cleaning supplies, snacks, etc.) prior to the start of all groups no less than 2 weeks prior to workshops.
  • Ensure meeting space for groups is reserved and submits Facilities requests to ensure room set up supports group activities.
  • Use approaches which are trauma informed, strengths-based, and culturally competent in interactions with patients and staff.
  • Attend and contribute to clinical, departmental, program and care team meetings and other organizational meetings and trainings as appropriate.
  • Liaises with outside groups, agencies, or individuals to coordinate education activities and arrange guest speakers.
  • Documents all patient encounters, including face to face, telephone, and electronic communications within two business days.
  • Ensures accurate and timely scheduling of group appointments in electronic medical records system (eCW) by self, Refugee Health Care Coordinators, and administrative staff.
  • Provides updates on program outcomes and patient stories for fundraising and communications purposes as needed.
  • Ensures staff distribute satisfaction surveys as groups conclude and enter data into required databases in the appropriate timeframe.
Requirements:
  • B.S. in Public Health or related field (social work, human services, family studies, psychology) or equivalent professional experience required.
  • At least 2 years of experience in the heath related field.
  • Experience facilitating educational groups and/or workshops required.
  • Experience with providing public health education to refugee populations strongly preferred.
  • Ability to work flexible hours including weekends and evenings required.
  • Commitment to work with underserved populations and ability to work in non-profit environments.
  • Demonstrated cultural competence in communicating with low-income populations.
  • Strong written and verbal communication skills.
  • Ability to work with computers, databases, Excel, and electronic health records.
  • Ability to speak Amharic, Dari, French, Haitian Creole, Pashto, Russian, Spanish, Tigrinya, and/or Ukrainian preferred.
