Job Title: Support Services Provider

Join Heartland Counseling Services as a Support Services Provider and make a meaningful difference in the lives of individuals impacted by mental health diagnoses, substance use diagnoses, or both.

About the Role:

This full-time position, 40 hours per week, may include Saturdays, working out of the SSC office. As a Support Services Provider, you will be responsible for providing direct contact with clients to develop skills necessary to live independently in the community.

Key Responsibilities:
  • Provide individual and group support by meeting with clients in their home, at Heartland Counseling Services, via transportation, or any other appropriate community setting.
  • Meet expectations for monthly billable baseline.
  • Provide support, screening, and intervention to the client in times of crisis and coordinate with crisis responders and others as necessary.
  • Participate in the on-call weekend rotation schedule to provide 24/7 crisis coverage.
  • Provide services at other HCS locations as assigned, which may include travel.
  • Actively teach skill-building concepts necessary to continue towards recovery in mental health and/or substance use.
  • Communicate and collaborate effectively with necessary outside individuals and agencies.
  • Provide community education services as assigned.
  • Demonstrate willingness to maintain flexibility and openness to new information through training and case consultation.
  • Establish hours of service delivery that ensures accessibility and responsiveness to the needs of the client.
  • Maintain up-to-date client charts to include needs assessment, treatment plan, progress notes, screenings, and discharge plans.
  • Attend scheduled supervisory and community-based staff meetings.
  • Attend scheduled client meeting observations with supervisor.
  • Complete all paperwork in accordance with agency policies and procedures.
  • Participate in community events as assigned.
  • Participate in a planning committee as assigned.
  • Always follow professional ethics.
  • Adhere to all agency policy and procedures.
  • Adhere to agency policy on confidentiality.
  • Perform other related duties as assigned.
Requirements:
  • Ability to form cooperative and supportive relationships with staff/agency.
  • Maintains ethics as dictated by profession.
  • Maintains certifications as needed.
  • Ability to form positive relationships with clients.
  • Strong interpersonal skills in working with diverse populations.
  • Skills in verbal and written communication.
  • Valid driver's license.
