Job Title: Registered Nurse - Case Manager

Matrix Providers is seeking a highly skilled Registered Nurse - Case Manager to join our team of dedicated healthcare professionals. As a Registered Nurse - Case Manager, you will play a vital role in providing exceptional patient care and support to our Military Service Members and their families.

Job Summary:
  • Participate in peer review and performance improvement activities to ensure high-quality patient care.
  • Function with an awareness and application of safety procedures to maintain a safe and healthy work environment.
  • Perform efficiently in emergency patient situations, following established protocols and remaining calm under pressure.
  • Participate in the implementation of the Family Advocacy Program, providing medical examinations, documentation, and reporting as directed.
  • Exercise awareness and sensitivity to patient/significant others' rights, as identified within the MTF.
  • Participate in continuing education to meet own professional growth and stay up-to-date with the latest medical advancements.
  • Attend and/or comply with all annual training classes required by the Command, including disaster training, infection control, and other relevant topics.
  • Actively participate in the command's Performance Improvement Program, identifying opportunities to improve patient care and recommending corrective action when necessary.
  • Provide in-service training to clinic staff members and support government employees, such as hospital corpsmen and students.
  • Perform timely, accurate, and concise documentation of patient care, operating and manipulating automated systems as prescribed by the Commander.
  • Exercise appropriate clinical delegation of tasks and duties, directing and coordinating health care team members, patient care, and clinical activities.
  • Maintain documentation of all treatment provided in accordance with clinic directives and prepare records and reports as required.
Requirements:
  • Degree: Bachelors Degree in Nursing.
  • Education: Graduate from a college or university accredited by Accreditation Commission for Education in Nursing (ACEN) and the Commission on Collegiate Nursing Education (CCNE).
  • Certification: Possess one of the following certifications: Commission for Case Manager Certification Certified Case Manager (CCM), Certification of Disability Management Specialists Commission: Certified Disability Management Specialist (CDMS), Association of Rehabilitation Nurses: Certified Rehabilitation Registered Nurse (CRRN), American Board for Occupational Health Nurses Certified Occupational Health Nurse (COHN) or Certified Occupational Health Nurse-Specialist (COHN-S), National Board for Certification in Continuity of Care: Advanced Certification in Continuity of Care (ACCC), Commission on Rehabilitation Counselor Certification: Certified Rehabilitation Counselor (CRC), American Nurses Credentialing Center Nurse Case Manager (RN-NCM), National Academy of Certified Care Managers: Care Manager Certified (CMC).
  • Experience: One year of clinical experience as a full-time RNCM within the last 3 years working in a medical facility.
  • Licensure: Current, full, active, and unrestricted license to practice as a Registered Nurse.
