We are seeking a skilled and compassionate Registered Nurse to join our team as a Home Hemodialysis Registered Nurse (HHD RN). As a key member of our healthcare team, you will be responsible for providing comprehensive patient care in an independent setting, empowering patients to take control of their health, and working collaboratively with patients, support systems, and care partners to achieve optimal outcomes.

Key Responsibilities
  • Provide patient education on Home Hemodialysis, enabling patients to administer their own dialysis and take ownership of their health.
  • Develop and implement individualized care plans, tracking patient progress and making adjustments as needed.
  • Collaborate with patients, families, and healthcare teams to ensure seamless care transitions and optimal patient outcomes.
  • Conduct home visits and on-call shifts, providing timely and effective support to patients and their families.
  • Participate in quality improvement initiatives, contributing to the development and implementation of best practices in patient care.
Requirements
  • Current Registered Nurse (RN) license in the state of practice.
  • Minimum of 9 months of RN experience.
  • Current driver's license, reliable transportation, and vehicle insurance in state of residence.
  • Associate degree in nursing (A.D.N) from accredited school of nursing required.
  • Basic computer skills and proficiency in MS Word and Outlook.
Preferred Qualifications
  • Bachelor of Science in Nursing (B.S.N); three-year diploma from accredited diploma program may be substituted for nursing degree.
  • Previous Chronic Hemodialysis, Acute Dialysis, Peritoneal Dialysis (PD), or Home Hemodialysis (HHD) nursing experience.
  • Training/Teaching/Healthcare Education background.
  • Previous case management experience.
  • Familiarity with, or excitement to learn about telehealth technologies.
  • Certified Nephrology Nurse (CNN), Certified Dialysis Nurse (CDN).
