Job Summary

We are seeking a highly motivated and organized Registered Nurse to join our team as a Home Hemodialysis Registered Nurse. As a Home Hemodialysis Registered Nurse, you will be responsible for providing comprehensive patient care in an independent setting, educating patients on Home Hemodialysis, and empowering them to take control of their health.

Key Responsibilities
  • Provide comprehensive patient care in an independent setting, applying core nursing skills to assess, diagnose, and implement complete care plans.
  • Track and organize patient cases, ensuring their treatment stays on track and witnessing the transformation of patients from feeling sick and afraid to strong, independent, and able to do daily activities they love.
  • Teach patients to administer their own dialysis, empowering them to take their health back into their own hands.
  • Work independently, building deep relationships with patients, support systems, and care partners, while wearing multiple hats as a Nurse, Coach, and Leader.
  • Manage your own cases, owning your own schedule, and working a typical 8-hour day Monday through Friday, with weekends and holidays off.

Requirements
  • Current Registered Nurse (RN) license in the state of practice.
  • Minimum of 9 months of RN experience.
  • Current driver's license, reliable transportation, and vehicle insurance in the state of residence.
  • Associate degree in nursing (A.D.N) from an accredited school of nursing required.
  • Basic computer skills and proficiency in MS Word and Outlook.
Nice-to-Haves
  • Bachelor of Science in Nursing (B.S.N); three-year diploma from an accredited diploma program may be substituted for nursing degree.
  • Previous Chronic Hemodialysis, Acute Dialysis, Peritoneal Dialysis (PD), or Home Hemodialysis (HHD) nursing experience.
  • Training/Teaching/Healthcare Education background.
  • Previous case management experience.
  • Familiarity with, or excitement to learn about telehealth technologies.
  • Certified Nephrology Nurse (CNN), Certified Dialysis Nurse (CDN).
