Job SummaryWe are seeking a highly motivated and compassionate Care Manager to join our team. As a Care Manager, you will play a critical role in providing care coordination services to individuals and families in need. Your primary responsibility will be to assess the needs of our clients and develop personalized care plans to address their unique challenges.
Key Responsibilities:- Provide care coordination services to individuals and families in need, including assessment, planning, and implementation of care plans.
- Maintain compliance with agency policies, applicable regulations, and relevant standards of care determined by accrediting/governing authorities.
- Plan, organize, and implement comprehensive services to consumers, including health, substance abuse, mental health, and social services.
- Demonstrate consistent and proficient knowledge, skills, and dispositions required to implement a coordinated approach to the delivery of services, linking consumers with appropriate services to address specific needs and achieve stated goals.
- Implement care management services to consumers addressing and coordinating services identified.
- Demonstrate understanding of contingency management models, theories of addiction, and related issues.
- Manage and distribute contingency management incentives to consumers who demonstrate positive behavior change consistent with meeting treatment goals.
- Complete timely DMHAS programmatic intake and follow-up surveys on each enrolled individual in the CM program.
- Submit timely monthly roster data and quarterly reports utilizing templates provided by DMHAS.
- Collect and report data timely of programmatic forms required at specific data collection points and ensure all data reported are accurate to meet SAMHSA obligations under GPRA.
- Recognize the importance of family, social networks, community systems, and self-help in recovery.
- Aware of insurance/health maintenance options and assist consumers in accessing those benefits.
- Rend care services through evidenced-based strategies to engage and retain consumers in care; provide linkages/referrals; coordinate, monitor, and follow up with care issues.
- Monitor consumer progress for changes in clinical symptoms and treatment side effects or complications.
- Accurately complete all required record keeping based on agency procedures.
- Maintain consumer confidentiality, adhere to related agency/program procedures.
- Must be able to assess and interpret data about the consumer's status in order to identify each consumer's needs and provide the appropriate care, including age-related care, needed by the consumer group.
- Demonstrate awareness, appreciation, and respect of cultural differences.
- Adhere to established policies, ethical guidelines by relevant governing authorities and contracts.
- Participate in all designated meetings and supervision sessions as well as clinical practice activities.
- Demonstrate ability to meet the organization's expectations for productivity, quality, and goal accomplishment.
- Follow all PI, HR, budget, contract requirements pursuant to the daily operations related to program.
- Education/Experience: Bachelor's degree in health, psychology, counseling, social work, education, or other behavioral health profession with 2 years' experience working in the field of addictions.
- Must possess a valid driver's license in good standing.
