Position Summary:The primary responsibility of the Wellness Coordinator is to oversee the daily medical and medication-related services offered to residents, ensuring compliance with all applicable federal, state, and local regulations, as well as adhering to the policies and procedures of Senior Housing Options.
Key Responsibilities:- Engage in the facility's quality management program as directed by the resident services director or administrator.
- Provide support and guidance to QMAPs and/or PCPs on medication-related matters.
- Orient and train new QMAP and PCP staff on medication room protocols and documentation.
- Monitor staff performance regarding medication room procedures and documentation, reporting any concerns to the Resident Services Director.
- Order and obtain medications and supplies as required to fulfill physician orders.
- Prepare medication administration records and medical file contents for new residents.
- Discuss medication room procedures with new residents, including appointment management and medication procurement.
- Update resident medication administration records as necessary for changes in physician orders.
- Maintain resident information records and other medical chart details as needed.
- Complete documentation and medication processing for resident transfers and discharges.
- Document in communication logs, resident chronological notes, and flow sheets in accordance with policy.
- Evaluate records to ensure complete documentation by QMAPs and PCPs.
- Assess residents for changes in condition and implement appropriate interventions.
- Notify family members or case managers of any changes in resident conditions.
- Communicate with physicians, diagnostic services, hospitals, and other external providers as necessary.
- Prepare medication reminder boxes in line with established policies.
- Assist residents in scheduling medical appointments and arranging transportation.
- Prepare necessary paperwork for physician appointments.
- May work as a QMAP or PCP to cover shifts as required.
- Obtain semiannual physician updates for each resident.
- Respect resident privacy and maintain confidentiality of resident information.
- Adhere to all safety protocols and emergency plans; complete accident/incident reports as needed.
- Uphold resident rights and report any concerns to a supervisor.
- Perform other duties as assigned.
- Involves sitting, standing, and walking throughout the day, with occasional driving.
- Utilizes office equipment such as computers, printers, and other devices.
- Must be able to lift up to 30 lbs comfortably.
- Comfortable using and climbing on step stools.
- Certain tasks may require repetitive movements, including climbing, crawling, crouching, pulling, and pushing.
- May be exposed to infectious waste and diseases.
This is a full-time position with a typical schedule of Monday through Friday, from 11:00 AM to 7:30 PM. Availability for off-hours may be required to address emergencies or unusual situations.
Qualifications:- Must possess patience, enthusiasm, and the ability to manage challenging resident interactions.
- Strong verbal and written communication skills, with the ability to provide clear, non-technical assistance.
- Demonstrated sensitivity to resident concerns and a commitment to fair treatment.
- Professional demeanor, positive attitude, and high ethical standards, even in challenging situations.
- Excellent organizational and interpersonal skills, with attention to detail.
- Intermediate computer skills, including proficiency in Microsoft Office Suite.
- Commitment to maintaining confidentiality of resident care information.
- Enjoys community engagement and resident interaction.
- Team-oriented approach, providing support across departments to ensure optimal resident care.
- Valid driver's license.
- Bilingual skills are a plus.
- High school diploma or equivalent required.
- Prior experience in a healthcare setting or with elderly populations preferred.
- Certification as a Qualified Medication Administration Person (QMAP) through an approved program is required.
- Ability to read, write, speak, and understand English.
- Proficient in Microsoft Office Suite, including Word, Excel, and Outlook.
