RN Care Coordinator

We are looking for a detail-oriented and compassionate RN to join our care coordination team. In this role, you will work closely with our interdisciplinary team to ensure seamless transitions between care settings and optimal resident outcomes.
Your responsibilities will include coordinating transfers, managing electronic health records, and communicating effectively with other healthcare professionals. You will also be involved in identifying opportunities for process improvements and implementing changes to enhance resident care.
