Key Responsibilities- Conduct comprehensive admission and discharge risk assessments to identify quality, financial, and patient satisfaction concerns.
- Negotiate and procure services and resources needed by patients, utilizing a multidisciplinary team approach.
- Monitor patient care and utilization of resources throughout the hospital stay, ensuring optimal outcomes and cost-effectiveness.
- Develop and implement transition care plans to secure appropriate post-hospital care arrangements.
- Participate in performance improvement initiatives and contribute to the development of evidence-based criteria tools.
- Masters in Social Work (MSW) degree required.
- Licensed Clinical Social Worker (LCSW) certification required.
- 3+ years of clinical experience in acute care settings, including internal medicine, surgery, and rehabilitation.
- Experience with utilization management, discharge planning, and third-party insurance review preferred.
- Certification in Basic Life Support (BLS) from the American Heart Association (AHA) required.
- State licensure in Massachusetts required.
