Key Responsibilities
- Develop and implement comprehensive plans of care for children and their families
- Collaborate with service providers to ensure seamless coordination of care
- Identify and address access issues, developing relationships with healthcare providers and tracking interventions and outcomes
- Work with children, families, and service providers to create a plan of care, identifying additional service providers as necessary and coordinating their involvement
- Manage electronic databases and navigate complex healthcare systems
Requirements
