About the Role

We are seeking a highly skilled Care Coordinator to join our team at Outreach Health. As a key member of our Care Team, you will play a vital role in delivering exceptional service to our clients and caregivers.

Key Responsibilities
  • Maintain high-quality service by adhering to Outreach Standard Operating Procedures, ensuring timely and accurate onboarding of new clients, and effective coordination of referrals, client welcome calls, data entry, eligibility checks, and caregiver matching.
  • Communicate effectively with referral sources to resolve client registration or authorization issues.
  • Ensure accurate and timely maintenance of clients' Electronic Health Records (EHRs).
  • Address client concerns with empathy and resolve issues promptly.
  • Support the Care Team to provide exceptional customer service and foster a collaborative environment.
  • Recruit and onboard caregivers, adhering to Outreach Standard Operating Procedures, and ensuring accurate and timely completion of new hire paperwork, reference checks, and orientation and training with Care Managers.
  • Communicate with case managers, practitioners, and others to facilitate client services and ensure continuity of care.
  • Assist in quality control and monitoring of caregiver master schedules.
  • Ensure accuracy, completeness, and timeliness of paperwork and assist with incoming and outgoing calls.
  • Monitor Electronic Visit Verification (EVV) Reason codes, enter client visits, and run checks and balances prior to submitting to payroll.
  • Generate timesheets, transmit to payroll, and reconcile billing.
  • Compute, record, and submit initial billing, conduct research on outstanding billings, and re-submit timely.
  • May reconcile Accounts Receivable (A/R) and Accounts Payable (A/P) reports.
  • May generate reports for use by Administrators and the Care Team.
  • Maintain confidentiality and adhere to Company policies and procedures with integrity.
Requirements
  • High School Diploma or GED.
  • Excellent organizational skills.
  • Effective verbal and written communication skills, with the ability to maintain accurate records.
  • High attention to detail and a high degree of accuracy.
  • Analytical skills for problem-solving.
  • Proficiency in MS Office Applications and ability to learn job-specific software systems.
  • Positive leadership skills, creating an environment of trust and a culture where everyone feels valued.
  • Humble, teachable, and open-minded.
Preferred Qualifications
  • At least 2 years of relevant work experience.
  • Bilingual (English and Spanish).
  • Experience with EHRs and tracking and monitoring Electronic Visit Verification (EVV).
  • Local knowledge of community health care and social welfare resources.
Compliance

The Care Coordinator is required to fully comply with applicable healthcare laws and statues (local, state, and federal) and Outreach Health policies and procedures. Must respect clients by recognizing their rights; maintaining confidentiality and work tirelessly to earn and keep their trust.

Pay Range: $27k-$32k (pay is hourly)
