Key Responsibilities- Conduct thorough assessments of members' needs and develop personalized care plans
- Collaborate with Care Managers, providers, and community organizations to address social determinants of health
- Develop and implement strategies to coordinate post-acute and long-term care services
- Monitor progress, evaluate outcomes, and report on care plan success
- Master's degree in Social Work
- Active and unrestricted LCSW or LMSW in NY
- Minimum 2+ years' experience in medical social work or case management
- Ability to travel 50-75% within Ulster/Orange County
