We are seeking Pharmacy Liaisons to fulfill a vital role within AndHealth. This position is based at our partner health center in Cambridge, OH.

Key Responsibilities:

  • Engage proactively with patients managing chronic health conditions to verify benefits and identify suitable secondary coverage options.
  • Conduct insurance verifications and medication coverage assessments, collaborating with healthcare providers to navigate formulary limitations.
  • Facilitate the completion of prior authorizations or appeals necessary for securing medication access for patients.
  • Maintain regular communication with patients regarding their medication usage and ongoing requirements, liaising with in-house or contracted pharmacies.
  • Perform routine evaluations of patients' medication usage and identify any concerns or side effects they may be experiencing.
  • Consult with pharmacists or healthcare providers on all clinical aspects of patient care, directing clinical interventions or counseling to the appropriate licensed professional.
  • Assist patients in scheduling prescriptions, refills, or deliveries.
  • Document all patient interactions within specialty pharmacy software.
  • Generate regular reports to identify new patients, upcoming medication needs, and adherence-related metrics.
  • Room patients attending in-person visits, collecting vital signs and updating relevant health information.
  • Ensure patients receive necessary lab work during visits, schedule follow-up appointments, and provide printed orders for additional tests.
  • Complete health questionnaires with patients as needed, utilizing tablets and other technologies.
  • Support patients in installing and accessing the AndHealth application on their mobile devices.
  • Manage patient referrals for specialty care.
  • Perform additional responsibilities as required.

Qualifications:

  • Bachelor's degree is preferred.
  • Certification as a Pharmacy Technician in the state of OH is required.
  • National certification (PTCB or ExCPT) is preferred.
  • Willingness to obtain and maintain licenses or certifications across multiple states is necessary.

Skills and Attributes:

  • Preferred experience of 3+ years in pharmacy or nurse navigation.
  • Exceptional written and verbal communication skills.
  • Strong attention to detail with the ability to manage multiple tasks simultaneously.
  • Comfortable with technology and adaptable to new systems.
  • Willingness to travel as required, with reliable transportation.
  • Organized and capable of working independently.

Additional Requirements:

  • Proof of negative TB skin test within the last 12 months.
  • Documentation of MMR immunity or a blood titer.
  • Documentation of Varicella vaccination or a blood titer.
  • Completion of Hepatitis B series.
  • Influenza vaccination is required.
  • COVID vaccination is required.
  • Additional health requirements may also apply.
