Role Responsibilities:

  • Engage proactively with patients managing chronic conditions to verify benefits and assist in identifying suitable secondary coverage options.
  • Conduct insurance verifications and medication coverage assessments, collaborating with healthcare providers to navigate formulary limitations.
  • Facilitate the completion of prior authorizations or appeals necessary for ensuring patient access to medications.
  • Maintain regular communication with patients regarding their medication usage and ongoing needs, liaising with in-house or contracted pharmacies.
  • Perform routine reassessments of patients' medication usage, identifying any concerns or side effects they may experience.
  • Consult with pharmacists or healthcare providers regarding the clinical needs of patients, ensuring all clinical interventions are directed to a licensed clinician.
  • Assist patients in scheduling prescriptions, refills, or deliveries as needed.
  • Document all patient interactions within specialty pharmacy software systems.
  • Generate regular reports to identify new patients, upcoming medication requirements, and adherence-related data.
  • Support in-person visits by rooming patients, collecting vital signs, and updating relevant health information.
  • Ensure patients receive necessary lab work during their visits, schedule follow-up appointments, and provide printed orders for additional tests.
  • Administer health questionnaires to patients when required, utilizing tablets and other technologies.
  • Assist patients in setting up and accessing the AndHealth application on their mobile devices.
  • Manage referrals for patients requiring specialty care.
  • Perform additional responsibilities as needed.

Qualifications:

  • A Bachelor's degree is preferred.
  • Certification as a Pharmacy Technician in the state of OH is required.
  • National certification (PTCB or ExCPT) is preferred.
  • Willingness to obtain and maintain licenses or certifications across multiple states is essential.

Desired Skills:

  • 3+ years of experience in pharmacy or nurse navigation is preferred.
  • Exceptional written and verbal communication abilities.
  • Strong attention to detail with the capacity to manage multiple tasks simultaneously.
  • Proficiency with technology and adaptability to new systems.
  • Willingness to travel as required, with reliable transportation.
  • Ability to work independently and maintain organization.

Additional Requirements:

  • Proof of negative TB skin test within the last 12 months.
  • Documentation of MMR immunity or a blood titer.
  • Proof of Varicella vaccination (2 doses) or a blood titer.
  • Completion of Hepatitis B series.
  • Influenza vaccination is required.
  • COVID-19 vaccination is required.
  • Additional health requirements may also apply.
