Role Responsibilities:
- Engage proactively with patients experiencing chronic health issues to verify benefits and identify suitable secondary coverage options.
- Conduct insurance verifications and medication coverage assessments, collaborating with healthcare providers to navigate formulary limitations.
- Assist in completing prior authorizations or appeals to secure medication access for patients.
- Maintain regular communication with patients regarding their medication usage and ongoing requirements, liaising with in-house or contracted pharmacies.
- Perform routine reassessments of patients' medication usage, identifying any concerns or side effects they may encounter.
- Consult with pharmacists or healthcare providers regarding patients' clinical needs, directing all clinical interventions or counseling to the appropriate licensed professional.
- Support patients in scheduling prescriptions, refills, or deliveries.
- Document all patient interactions in specialty pharmacy software systems.
- Generate reports to track new patients, upcoming medication requirements, and adherence-related metrics.
- Assist with patient check-ins during in-person visits, collecting vital signs and updating relevant health information.
- Ensure patients receive necessary lab work during visits, schedule follow-up appointments, and provide printed orders for additional tests.
- Facilitate the completion of health questionnaires with patients as needed, utilizing tablets and other technologies.
- Guide patients in installing and accessing the AndHealth app on their mobile devices.
- Manage patient referrals for specialty care.
- Perform additional responsibilities as required.
Education & Licensure Requirements:
- Bachelor's degree is preferred.
- Certified Pharmacy Technician in the state of OH is required.
- National certification (PTCB or ExCPT) is preferred.
- Willingness to obtain and maintain licenses or certifications across multiple states is essential.
Preferred Skills and Qualifications:
- 3+ years of experience in pharmacy or nurse navigation is preferred.
- Exceptional written and verbal communication skills.
- Strong attention to detail with the ability to manage multiple tasks simultaneously.
- Comfortable with technology and adaptable to new systems.
- Willingness to travel as needed; reliable transportation is required.
- Organized and capable of working independently.
Additional Health Requirements:
- Proof of negative TB skin test within the last 12 months.
- Documentation of MMR immunity or a blood titer.
- Documentation of Varicella vaccination or a blood titer.
- Completion of Hepatitis B series.
- Influenza vaccination is required.
- COVID vaccination is required.
- Additional health requirements may apply.
