We are seeking a highly skilled and compassionate Primary Care Nurse Practitioner to join our team at HarmonyCares. As a Primary Care Nurse Practitioner, you will be responsible for providing high-quality, patient-centered care to our patients in the comfort of their own homes.
Responsibilities:
- Provide primary care to high-acuity patients in their homes, Monday through Friday
- Work collaboratively with our interdisciplinary team to develop and implement individualized care plans
- Conduct thorough assessments, diagnose, and treat patients with a range of medical conditions
- Order and interpret diagnostic tests, prescribe medications, and provide education to patients and their families
- Participate in quality improvement initiatives to ensure the highest level of patient care
Requirements:
- Current licensure as a Nurse Practitioner in the state where HarmonyCares Medical Group is located
- Federal DEA license/eligibility
- Geriatric training/experience preferred
- Active CPR/BLS certification
