Job Summary:
Montefiore Medical Center is seeking a highly skilled and experienced Registered Nurse (RN) to join our Home Health team as a Nurse Manager. As a key member of our team, you will be responsible for providing clinical oversight and supervision to a staff of Certified Home Health Aides (CHHAs) and Registered Nurses (RNs).
Key Responsibilities:
- Supervise and manage a team of CHHAs and RNs to ensure high-quality patient care and services.
- Provide clinical oversight and guidance to ensure compliance with regulatory requirements and Montefiore's policies and procedures.
- Develop and implement effective care plans that meet the unique needs of each patient.
- Collaborate with other healthcare professionals to ensure seamless care transitions and coordination.
- Conduct regular performance evaluations and provide feedback to staff members.
- Identify and address operational issues that impact patient satisfaction and quality of care.
Requirements:
- 2 years of experience in Home Health care with a focus on CHHA supervision.
- 1 year or more of supervisory experience in a healthcare setting.
- Ability to educate and motivate staff to provide high-quality patient care.
- Excellent communication and interpersonal skills.
- Knowledge of Medicare conditions of participation and regulatory requirements.
- BSN (Masters preferred) with a valid NYS RN license and registration.
- Valid driver's license and ability to provide own transportation during working hours for the purpose of making home visits.
