Responsibilities- Provide personalized care navigation services to patients and their families, ensuring seamless transition from hospital to home or other care settings.
- Assess patients' needs and develop individualized care plans that address their unique requirements and goals.
- Collaborate with healthcare providers, patients, and families to ensure effective communication and coordination of care.
- Bachelor's degree in a healthcare-related field or equivalent experience.
- At least 3 years of experience in care management or a related field.
- Current licensure as a registered nurse (RN) or licensed vocational nurse (LVN)/licensed practical nurse (LPN).
The estimated salary for this role is between $72,000 and $95,000 per year, depending on experience. In addition to competitive pay, we offer a comprehensive benefits package, including medical, dental, vision, and life insurance, as well as a 401(k) retirement plan and paid time off.
